AlloDerm Experiences of Major Burn Patients: AlloDerm Can Prevent Post Burn Joint Contracture

Abstract
Purpose: AlloDerm (LifeCell Co., NJ) is derived from donated human skin. After removing epidermis and cells, it preserved with a freeze drying process. It become a framework of biochemical and structural component of dermis. To prevent joint contraction in major burns, we have tried the acellular human dermis (AlloDerm¨Þ) in acute burn stage.

Methods: Included in this study were 64 subjects who had sustained major burns from March, 2005 to July, 2007. The operative method used was as follows: 1) Early wound excision and cadaver skin coverage. 2) 2¡­3 weeks later, skin graft was performed with AlloDerm and CEAs (in case of using CEA, wide meshed was performed). Follow up of 31 patients among them from January, 2008 to March, 2008 to find out the functional results by measuring ROM of AlloDerm used joints.

Results: The average burn area was 36.8% TBSA. The average length of days from admission to last skin graft was 36.6 days (18¡­63). The average number of operation was 2.78 (1¡­5). Rehabilitation Medicine results was as follows: 1) 16 knees of 10 patients, average knee joint ROM was 129.1 degree (full knee ROM is 150 degree), 2) 24 elbows of 16 patients, average ROM was 293.1 degree (full ROM is 310), 3) 9 shoulders of 8 patients, average ROM was 485 degree (full ROM is 500), 4) 6 wrist of 3 patients, average ROM was 180 degree (full ROM is 180), 5) 4 hands of 2 patients were showed no functional limitation of motion.

Conclusion: AlloDerm¨Þ can prevent post burn joint contracture successfully in acute stage of major burns. (Journal of Korean Burn Society 2008;11:44-49)
